

TERMS OF SERVICE
Rehab & Restore Physiotherapy

These terms explain the arrangements for appointments and physiotherapy services provided by Christabel Entwistle, trading as Rehab & Restore Physiotherapy.
By confirming an appointment after being provided with these terms, you agree to them.
Appointments
Appointments are offered at the agreed clinic location or as a pre-arranged home visit. Appointment length and fees will be explained when you book.
Please arrive promptly for clinic appointments and be available at the agreed time for home visits. If you arrive late, the appointment may need to finish at the original scheduled time so that other patients are not delayed. The full appointment fee will still apply.
Fees and payment
The fee will be confirmed before your appointment. Any additional home-visit travel charge will also be agreed in advance.
Payment is by bank transfer and is due on the day of the appointment unless otherwise agreed. Payment details will be provided when the appointment is confirmed.
Rehab & Restore Physiotherapy may change its fees, but any appointment already booked will be charged at the price confirmed when booking.
Charges for late cancellations or missed appointments will be invoiced separately in accordance with the cancellation policy below.
Cancellations and missed appointments
Please provide at least 24 hours’ notice if you need to cancel or rearrange an appointment.

Cancellations made with at least 24 hours’ notice will not be charged.

Cancellations made with less than 24 hours’ notice will be charged at 50% of the appointment fee.

If you do not attend an appointment without providing notice, the full appointment fee will be charged.
Exceptional circumstances will be considered at the discretion of Rehab & Restore Physiotherapy.
If Rehab & Restore Physiotherapy needs to cancel your appointment, you will not be charged and an alternative appointment will be offered wherever possible. Rehab & Restore Physiotherapy is not responsible for other costs resulting from an appointment having to be cancelled.
Assessment, treatment and consent
Your first appointment will normally include a discussion about your health, symptoms, goals and relevant medical history, followed by an appropriate physical assessment.
The proposed treatment, its expected benefits and any material risks or alternatives will be discussed with you. You may ask questions, decline any part of an assessment or treatment, or withdraw your consent at any time.
Treatment recommendations are based on the information available at the time. Please provide accurate information and let me know about any relevant changes to your health, medication or symptoms.
Physiotherapy outcomes vary between individuals and cannot be guaranteed.
Your responsibilities
You are responsible for:

Providing accurate and relevant health information.

Informing me of changes to your health or medication.

Following agreed advice and exercises safely.

Stopping an exercise and seeking advice if it causes unexpected or concerning symptoms.

Seeking urgent medical assistance when necessary.
Rehab & Restore Physiotherapy does not provide an emergency service. If you require urgent medical attention, contact NHS 111, your GP, or call 999 in an emergency.
Home visits
For a home visit, you must provide a safe and suitable environment for assessment and treatment. Please advise me beforehand of relevant access difficulties, parking restrictions, pets, smoking within the property or other potential safety concerns.
A home visit may be ended or rearranged if the environment is considered unsafe or unsuitable.
Children and young people
For patients under 18, appropriate consent will be obtained from the young person and/or a person with parental responsibility, according to the patient’s age and ability to understand the proposed care.
A parent, guardian or responsible adult may be required to attend appointments.
Communication between appointments
Reasonable contact about your treatment or exercises may be offered between appointments. This is not an emergency or continuous clinical advice service, and responses may not be immediate.
Personal or detailed medical information should not be sent through social media.
Clinical records and privacy
Accurate and confidential clinical records will be maintained in accordance with legal and professional requirements.

Professional standards
Physiotherapy is provided by an HCPC-registered physiotherapist who is a member of the Chartered Society of Physiotherapy and holds appropriate professional liability insurance.
Treatment will only be provided where it is considered appropriate and within professional scope of practice. Where necessary, you may be advised to seek assessment from your GP, another healthcare professional or an emergency service.
Rehab & Restore Physiotherapy may decline or discontinue treatment where it is not clinically appropriate, falls outside its scope of practice, professional boundaries are not respected, fees remain unpaid or there is a safety concern. Wherever appropriate, this will be explained and alternative support suggested.

Governing law
These terms are governed by the laws of England and Wales. Nothing in these terms affects your statutory consumer rights.
